New Patient Registration
We welcome new patients from within the practice area, please use the links in Registering Online to complete the Request to Register form and Health Questionnaire - these are also available to collect from the surgery reception.
All patients registered at Lightwater Surgery have a named GP.
Dr Paramore is currently the named GP for new patients registering with Lightwater Surgery.

We will also accept patients from the Upper Longcross area.
As it takes several weeks for your records to arrive, you will be asked to complete a Health Questionnaire giving us essential details of your health record. Both forms needed for registration are available on the “Registering Online” tab.
We also like new patients over the age of 5 to have a new patient check with the practice health care assistant. This is a simple, often valuable check which can also serve as a useful introduction to the practice.
