New Patient Registration
We welcome new patients from within the practice area; to register please bring in your current medical card, (if available), or complete form GMS1.
All patients registered at Lightwater Surgery have a named GP. Dr Wilcox is currently the named GP for new patients registering with Lightwater Surgery.
Our practice area covers the following postcodes:-

We will also accept patients from the Upper Longcross area.
As it takes several weeks for your records to arrive, you will be asked to complete a questionnaire giving us essential details of your health record. Both forms are available on the above “Registering Online” tab.
We also like new patients over the age of 5 to have a new patient check with the practice health care assistant. This is a simple, often valuable check which can also serve as a useful introduction to the practice.
Change of Details
Please notify the surgery of any change to your name, address or telephone number. Should you move outside our practice area we may ask you to register with a doctor nearer to your home. Please do not delay over this.
Patients can update their details by completing a change of details form and taking it to the surgery.
Registering For Online Services
We encourage patients to register for our online services. This will allow patients to book appointments as well as order repeat prescriptions without having to wait on the phone to speak to the next available recpetionist. Additionally, patients are able to view aspects of their clinical record and any recent blood test results. Online services are available 24 hours a day 7 days a week.
Patients can register either by speaking to reception and completing the required registration forms or by downloading the NHS App. You can register to use online services through the NHS App without having to come to the surgery.
